Finding Doctors and Facilities
Having Flexible Choice makes it easy to find doctors and facilities for your care. You can choose your provider and where to get care with three options: HMO (Option 1), PHCS & MultiPlan Networks within MAS and other Kaiser Permanente states or Cigna Healthcare PPO Network when travelling outside Kaiser Permanente states (Option 2) and Out-of-Network (Option 3). You can switch between provider options as you desire.

HMO
Option 1: Kaiser Permanente physicians and medical centers.
- Access any Kaiser Permanente provider, affiliated hospital, or pharmacy, all within the integrated delivery system.
- There are over 1,500 specially selected physicians in the Permanente network. To help you find a primary care physician who’s right for you, browse our online doctor profiles.
- You also have access to more than 30 medical centers across the region. Check here for services, hours and locations of each medical center—including urgent care.
PHCS & MultiPlan Networks or Cigna PPO Network
Option 2: Participating PHCS & MultiPlan Networks or Cigna PPO Network providers.
- Through Option 2, you can call or make an appointment directly with any participating provider. To find up-to-date Participating Providers or confirm if your current provider is part of the Network, you can search for Providers by Location, or call Customer Service at 1-888-225-7202 (TTY 711), Monday through Friday 8:00 a.m. to 6 p.m., Eastern time.
Kaiser Permanente States: Within CA, CO, GA, HI, MD, OR, VA, WA and DC you have access to Flexible Choice Provider Directory
Non-Kaiser Permanente States: The Cigna Healthcare PPO Network* only provides access to care in non-Kaiser Permanente states.
*Disclaimer: The Cigna Healthcare PPO Network refers to the health care providers (doctors, hospitals, specialists) contracted as part of the Cigna Healthcare PPO for Shared Administration.
- No referral is needed for office visits to Option 2 physicians or specialists.
- It is always a good idea to call the provider’s office to confirm that they’re a participating provider.
Can I nominate a physician to become an Option 2 provider?
- Yes. If you were seeing a doctor who is not a member of either the PHCS or MultiPlan networks within the Kaiser Permanente States of CA, CO, GA, HI, MD, OR, VA, WA and DC or a Cigna Healthcare PPO Network provider outside of the Kaiser Permanente States before you became a Flexible Choice member, you can nominate that doctor to join our network. Please keep in mind your provider may choose to join our network.
- To nominate a PHCS provider in a Kaiser Permanente state, visit (keep link) for information or call the MultiPlan provider information line at 888-220-6010. To nominate a Cigna PPO Network provider outside of Kaiser Permanente states, call Customer Service at 1-888-225-7202 (TTY 711), Monday through Friday 8:00 a.m. to 6 p.m., Eastern time.
- Review can take up to 6 months Whether a physician or practitioner is accepted depends on a number of considerations, including the provider’s interest in joining, so a nomination doesn’t guarantee acceptance.
Out-of-Network
Option 3: Choose any licensed provider anywhere for care.
- Through Option 3, you can work directly with any licensed provider or facility anywhere.
- No referral is needed for office visits to Option 3 physicians or specialists.
- Just remember that this option generally has the highest out-of-pocket costs.
