Overview

Added Choice and Deductible Added Choice are point-of-service (POS) products that combine an in-network HMO provider option and an out-of-network provider option in one plan.

Members can move among the two provider options at any time, so they can always make the best decisions for their health and their wallets.

In-Network Providers

Added Choice offers the Kaiser Permanente SignatureSM Provider Network or Kaiser Permanente SelectSM Provider Network for the in-network option.

  • With Kaiser Permanente’s Signature Provider Network, members can access highly rated integrated healthcare delivery system. This option provides the most coordinated care with the lowest out-of-pocket costs.
  • If the Kaiser Permanente Select Network is chosen, members can also access many community physicians in private practice.

Out-of-Network

Any other licensed provider or physician.

  • Members can receive services from any licensed provider or facility they choose (except those already considered in-network).

 

Deductibles, coinsurance and copays for options.

Added Choice No deductible. Most services are covered at a copay. Most services are subject to a deductible, and then coinsurance.
Deductible Added Choice (Available for small size groups only) Some services are subject to a deductible*, then copay or coinsurance.

Certain services are covered before the deductible at a copay.

Most services are subject to a deductible*, and then coinsurance.

*Each option has a separate deductible. If a member meets the deductible in one option, they will pay only the applicable copay or coinsurance for covered services received in that option. If they receive services in another option, they must meet that option’s deductible amount before only paying the applicable copay or coinsurance for covered services received in that option.
